When you arrive, please turn off all electronic devices (e.g., cell phone).
new guests
Please arrive 15 minutes prior to your appointment to complete required paperwork.
cancellation policy
We understand that schedules change, however, we do require a 24-hour notice of cancellation or appointment rescheduling for all services. Changes within the 24-hour period will result in a 100% charge for the scheduled service.
forms of payment accepted
We accept MasterCard, Visa, cash, and personal checks (a $20.00 fee will be charged for all returned checks).
gift certificates
Purchase gift certificates from the health center. Lost, misplaced, or stolen gift certificates cannot be replaced. Gift certificates are non-refundable and may not be used for cash or gratuities.
